Manifestaciones orales en pacientes con enfermedades reumatológicas, atendidos en la consulta de reumatología de una clínica privada en la ciudad de Managua 2022-2023

Resumen

Las enfermedades reumatológicas se caracterizan por la pérdida de la tolerancia inmunológica, es decir, la capacidad del cuerpo de auto reconocerse. Existen diferentes mecanismos que están implicados en su fisiopatología, como la alta producción de autoanticuerpos y reactantes de fase aguda, mismas que pueden producir daño directo a nivel sistémico, entre estas, se encuentran: lupus eritematoso sistémico (LES) artritis reumatoide (AR) y síndrome de Sjögren (SS). Objetivo: Determinar la prevalencia de las manifestaciones orales en pacientes con enfermedades reumatológicas y describir las características sociodemográficas. Material y método: Se llevó a cabo un estudio descriptivo, con paradigma mixto, con enfoque secuencial exploratorio, mediante una revisión documental de historias clínicas de pacientes con enfermedades reumatológicas y mediante la aplicación de una entrevista a una especialista en reumatología. Se realizó un análisis cualitativo de las categorías investigadas. El total de historias clínicas corresponde a 83, de las cuales 36 cumplieron los criterios de inclusión, mismas diagnosticadas con enfermedades reumatológicas por la subespecialista Dra. Deyanira Reyes, en una clínica privada. Conclusión: La prevalencia de las manifestaciones orales en pacientes con afecciones reumatológicas es del 43%. Donde el 97% son femeninas y 3% masculino. Con mayor recurrencia entre la segunda y quinta década de vida. Las enfermedades reumatológicas con mayor frecuencia fueron Fibromialgia 33%, Lupus eritematoso sistémico (LES) 23%, artritis reumatoidea (AR) 19%, síndrome de Sjögren (SS). De las manifestaciones orales más frecuentes en la población de estudio se incluyeron la xerostomía (sequedad bucal) 57%, úlceras aftosas 18%, disfunción temporomandibular DTM 10%, la disfagia 10%; y periodontitis 5%.
